A 50 years old male patient with a chief complaint of Vomiting’s and body weakness

A 50 years old male patient, Farmer by occupation, resident from Tripathi, came to the department with a chief complaint of :-

Vomiting’s with body weakness.


History of present illness :-

The patient was apparently asymptomatic in  1 year ago. Then he suddenly had Vomiting’s for 3 days   3-4 times a day with body weakness. Then he used few antibiotics. Even then there was insidious onset gradual progression body weakness and leg swelling. Then they consulted nearly hospital and under hon few tests the got to know about his kidney failure. They came to our hospital for dialysis treatment.


History of past illness :-

No history of similar complaints in the past.

K/H/O - Hypertension 

NK/H/O - Diabetes mellitus, Tuberculosis, Epilepsy, CAD, CVD, Asthma.


Family history :- 

No significant family history.


Personal history :-

Mixed diet.

Normal appetite.

Normal sleep cycle.

Regular vowel and bladder 


General examinations :-

Pallor is present.

No Icterus.

No Cyanosis. 

No clubbing. 

No lymphadenopathy. 

Pitting Edema is present.

Vitals :- 

Temperature :- Afebrile 

BP :- 130/80
