A 64 years old male patient with a chief complaint of fever, leg swellings and difficulty in breathing.

A 64 years old male patient, farmer by occupation, resident of Silpuram, came to the hospital with a chief complaint of 

Fever, leg swellings and difficulty in breathing.


History of present illness:-

Patient was apparently asymptomatic 1 year ago and developed fever for two days and later which was on and off type , then leg swellings and difficulty in breathing on walk of short distance. He went to bear by hospital and undergone few tests and resulted with kidney failure.


History of past illness :-

No history of similar complaints in the past.

Know history of Hypertension, Diabetes mellitus.

Diabetes mellitus is an aggregating factor.


Family history :-

Know family history of Diabetes mellitus.


Personal history :-

Eating vegetarian since 1 year

Loss of appetite 

Disturbed sleep 

Irregular bowel and bladder movements 

No smoking and alcohol 


Drug and allergic history:-
No known relevant drug and allergic history


General examinations :-


Patient is conscious, coherent, cooperative and well oriented to time, place and person.
No Pallor
Light Icterus is present 
No Clubbing
No Cyanosis
No Lymphadenopathy
Edema of feet present 
No Malnutrition
